What a Sports Physical Covers
A pre-participation physical evaluation is a focused check that it’s safe for you to train and compete. The most important part isn’t the exam — it’s the history. Fainting during exercise, chest pain, unexplained shortness of breath, prior concussions, or a family history of sudden cardiac death before age 50 are the findings that change what happens next, and they only come up if someone asks.

Massachusetts Requirements
Under MIAA rules, a physical exam is good for 13 months — a physical dated April 1st covers a student through May 1st of the following year. If it lapses mid-season, the student is ineligible until a new one is on file, so it’s worth checking the date on your last form before tryouts rather than the week of.
Individual schools and leagues sometimes add their own paperwork on top of the state requirement. Bring whatever they gave you and it will be completed at the visit.
How It Works
- Book a visit. Sports physicals are done in person — the exam is the point.
- Complete the sports intake form. Filling it out ahead of your visit expedites the process.
- Bring your form and records. The school or league form, immunization records, and a list of any medications.
- Come in for the exam. Most visits take about 30 minutes.
- Leave with it signed. The form is completed and signed before you go. If anything needs follow-up, we’ll walk you through it.

Frequently Asked Questions
No. New patients are welcome for sports physicals. If you’d like Dr. Mui to be your primary care physician going forward, we can set that up at the same visit.
They overlap, but they aren’t the same thing. A sports physical is built around safe participation; an annual check-up is broader — screening, labs, immunizations, mental health, and prevention. If you want both covered in one visit, say so when you book and we’ll schedule the time for it.
Routine EKG screening isn’t required for most young athletes, and major U.S. guidelines don’t recommend it universally. It becomes appropriate when the history or exam raises a question — fainting with exertion, chest pain, a murmur, or a family history of sudden cardiac death. If it’s indicated, we can do it in the office for $50.
For patients under 18, yes — a parent or guardian needs to be present to consent and to help with the family history, which is the part of the form athletes most often can’t answer on their own.
Most athletes are cleared without restriction. Occasionally clearance is deferred pending an EKG, an echocardiogram, or a cardiology or orthopedic opinion. That’s not a no — it’s a pause while we sort it out, and we’ll help arrange the next step quickly so you don’t miss more of the season than necessary.
