Many travelers, students, and employees need a medical certificate signed by a licensed physician before they can finalize a visa, enroll in a study abroad program, or board a flight. Dr. Mui regularly completes these documents for patients heading anywhere in the world.

Official Physician Stamp
Most consulates, embassies, schools, and airlines require a physician’s signature and an official physician stamp. Documents missing the stamp are often rejected and have to be redone — a costly delay when you’re close to a travel date.
Every certificate Dr. Mui completes is signed and sealed with his official physician stamp, so your paperwork is accepted on first submission.
International Health Regulations (2005)
Some destinations require vaccination documentation that complies with the World Health Organization’s International Health Regulations (2005). The most familiar example is the International Certificate of Vaccination or Prophylaxis (ICVP) — also known as the “Yellow Card” — commonly required for yellow fever and used to record other travel-related immunizations.
Dr. Mui completes the ICVP in the WHO-approved format with all required fields, signature, and stamp. If you’re traveling to a country that requires proof of yellow fever vaccination specifically, please mention this when scheduling so we can review your destination’s requirements together.
How It Works
- Schedule a visit. A short office visit is usually all that’s needed. Telehealth may be appropriate for follow-up paperwork or letters that don’t require a physical exam.
- Complete the Medical Intake Form. Fill in your medical history, current medications, and travel details ahead of time so the visit goes faster.
- Complete the New Patient Form. For new patients only.
Frequently Asked Questions
No. New patients are welcome for travel medical certificates. If you’d like Dr. Mui to be your primary care physician going forward, we’re happy to set that up too.
Most straightforward forms can be completed during a single office visit. If labs, vaccinations, or specialty records are required, we’ll let you know up front so you can plan around your travel date. Call or text 617-675-4085 for time-sensitive requests.
Please bring a valid photo ID, the specific form your destination requires (if any), your vaccination records, and a list of current medications. If you have your existing “Yellow Card” from a previous trip, bring that as well so we can update it.
Travel-related paperwork is typically not covered by insurance, since it’s considered an administrative service rather than medical care. We’ll discuss any fees with you before the visit so there are no surprises.
It depends on what the form requires. Letters that don’t require a physical exam can sometimes be completed by telehealth. Forms that require an in-person exam, vaccinations, or an official ICVP stamp typically require a brief office visit.
